Location: 
Derby (Multiple sites)
 •    South Derbyshire - Out of Hours Bases (Long Eaton, Ilkeston Hospital, St Oswald’s, Swadlincote Health Centre, and Derby Urgent Treatment Centre) .
•    Out of Hours Home Visiting (based from Derby Urgent Treatment Centre, Ilkeston and Orbis).
 •    Erewash – Acute Home Visiting.
 •    Royal Derby Hospital.

About the role:
The post holder will be responsible for the autonomous clinical assessment, treatment and/or referral or giving advice as appropriate to first contact patients across a wide spectrum of clinical conditions within the services provided by DHU Health Care CIC through direct contact consultations, but be based in a team based area with support from Advanced Practitioner and or GP's.
 
A commitment to a quality service, excellence in clinical practice and continuous professional development are integral to the role and the development of the service.
If desired, this post is a developmental position with funded Advanced Clinical Practitioner training and support available. 

Where applicable, the post holder will be given the opportunity to work across the wider DHU Health Care CIC services to gain experience. The post holder will participate in promoting and advancing DHU Health Care CIC, whilst working towards DHU’s Values, behaviours and principles.

Responsibilities:
    To practice autonomously without direct supervision, but supported by other clinicians is always available if needed.
    To utilise advanced clinical skills to assess, diagnose, treat and refer patients within the clinical service.
    To recognise the need for, initiate and where appropriate interpret relevant diagnostic investigations.
    To recognise the need for and initiate where appropriate advice of relevant specialisms.
    To exhibit excellent non-verbal and verbal communication skills when dealing with patients and carers. To have the ability to make clear decisions with confidence and communicate these ef-fectively.
    To maintain appropriate clinical records in line with current practice guidelines.

Essential Qualifications:
    Registered Adult Nurse registered with the Nursing & Midwifery Council (NMC), Paramedic registered with the Health Care Professions Council (HCPC) or Pharmacist registered with the General Pharmaceutical Council (GPhC) plus additional evidence of Autonomous Practice Qualifications (not including degree modules) such as;
    Level 6 or 7 Clinical Examination Module.
    Level 6 or 7 Minor Illness Module .
    RCN Accredited Nurse Practitioner Module.
    Emergency Care Practitioner Qualification.
    Autonomous practice A33.

Desirable Qualifications:
    BSc (Hons)
    Mentorship qualification
    Registration as an independent non-medical prescriber

Essential Skills:
    Can complete a concise clinical history and physical examination.
    Can formulate a differential diagnosis and management plan.
    Prepared to participate in service development.
    Demonstrable awareness of accountability in clinical practice.
    Can give evidence of using effective communication skills within practice.

 •    IRMER & X-Ray interpretation (only at Royal Derby Hospital UTC).
 •    Pathology interpretation.
 •    Telephone triage.
